Volunteer U18 Parental Consent

Under 18 Parental Consent Form

Your son/daughter has applied to volunteer with BCM International NI. Since they are under 18 we require your parental consent in order to proceed with the application process. Your son/daughter’s application will not be considered until the BCM NI Office has received this form.

For applications relating to Mullartown House

Please note that should your child leave Mullartown House premises during free time (ie not a camp related activity) they will not be covered by BCM NI’s insurances. This applies whether they are on foot, travelling in their own car, or travelling in another volunteer’s car.

For applications for Hello Life

Please note that your son/daughter may be transported in other volunteers’ cars during the week of Hello Life. If your son/daughter brings a car to Hello Life they may be asked to transport other team members or equipment during the week. If you have any concerns regarding this, please email nate.heater@bcmni.org

Drivers will ensure that they:

Are suitably competent and qualified for the vehicle they are using.
Know what to do in the event of a breakdown or accident.
Have adequate and up to date insurance cover to carry under 18s in their car.
Drive carefully and within national speed limits.
Ask passengers to wear seatbelts.
Are police checked.
Have a roadworthy vehicle which provides a single seat and seat belt for each child/young person.

I give permission for my child’s volunteer application for the above named camp or ministry to be considered. If offered a volunteering opportunity with BCM NI I give permission for them to be contacted by text or email by their BCM Team Leader. In the event of an emergency I authorise their BCM Team Leader to give permission to the medical authorities to undertake whatever treatment is considered necessary.
